Why use a fixed sequence instead of eyeballing the tracing?
Pattern-matching makes beginners fixate on dramatic findings (like ST changes) and miss quiet killers such as complete heart block or a long QT. A checklist forces you to comment on every domain.
Normal ranges: ventricular rate, PR, QRS, QT
Rate 60–100 bpm; PR 120–200 ms (3–5 small boxes); QRS <120 ms (<3 small boxes); QT roughly less than half the R–R at normal rates (use QTc for precision).
What three questions confirm sinus rhythm?
Is there a P before every QRS? A QRS after every P? Is the P upright in lead II and inverted in aVR?
Regularly irregular vs irregularly irregular
Regularly irregular has a repeating pattern (e.g., Wenckebach); irregularly irregular has no pattern at all, classically atrial fibrillation.
How do you quickly determine axis?
Look at leads I and aVF: both positive = normal axis; other combinations sort you into left, right, or extreme deviation.
Signs of atrial enlargement on the P wave
Right atrial enlargement: tall peaked P >2.5 mm in lead II. Left atrial enlargement: wide, notched P in II or deep terminal negative deflection in V1.
Sokolow–Lyon criterion for LVH
S wave in V1 + R wave in V5 or V6 ≥ 35 mm, often with a strain pattern.
What are the anatomic lead groups for ischemia?
Inferior: II, III, aVF. Lateral: I, aVL, V5, V6. Anterior/septal: V1–V4. Changes should be territorial and reciprocal to be convincing.
Why is comparison to an old EKG step 8?
"New" is the most powerful adjective in cardiology — 2 mm of ST elevation present for ten years is far different from one that appeared this morning.
How should you treat the computer's auto-read?
As a hypothesis, not a diagnosis. Write your own interpretation first; machines are especially unreliable for rhythm and axis.
